Abstract

Objectives: To evaluate which factors are predictive for unsuccessful pessary fitting. Methods: This cross-sectional study recruited women ≥55 years with one or more pelvic floor symptoms in twenty general practices in the Northern part of the Netherlands. Women who did not meet the exclusion criteria and screened positive for ≥1 pelvic floor symptoms completed a validated questionnaire and were invited for physical examination. Those diagnosed with a moderate prolapse (at or beyond the hymen, POPQ stage IIB-III) were included in this study. All women were fitted a pessary and the success was evaluated after 2 weeks. The pessary fitting was accounted as unsuccessful if the pessary fit failed at three attempts. Results: Mean age was 64 years. Within the 81 participants who were fitted a pessary, 34 women had an unsuccessful fit (42%). The most prolapsed compartment being a rectocele (OR=4.03/95%BI=1,12-14,5), familiar predisposition for prolapse (OR=3,29/95%BI =1,10-9,85), being sexually active (OR=4,09/95%BI=1,34-12,5) a higher birth weight of the heaviest child (OR=1,78/95%BI=1,13-2,79), a higher score of the Pelvic Floor Distress Inventory Questionnaire (PFDI-20) (OR=1,02/95%BI=1,00-1,03), and incontinence of loose faeces (OR=3,02/95%BI=1,08-8,44) were predictors for unsuccessful pessary fitting. Conclusions: A rectocele, familiar predisposition for prolapse, being sexually active, a higher birth weight of the heaviest child, more pelvic symptoms (higher PFDI-20 score), and incontinence of loose faeces were predictors for unsuccessful pessary fitting in women aged ≥ years with advanced pelvic organ prolapse.
