Abstract

Background: Previous studies have shown the use of intra-operative instructional video’s to have a positive effect on learning laparoscopic procedures. In the current study, we investigated the effect of the point in time, at which the instructional video was shown. The secondary goal was to explore the transferability of skills from one type of laparoscopic simulator to another. Methods: In this explorative study, medical student and residents, with limited experience operating laparoscopic instruments, were randomised into two groups. After completing a basic skills course on a virtual reality simulator, both groups performed a standardised task four times on the TrEndo augmented reality simulator, using an instructional video, shown either pre-operatively or intra-operatively, respectively. During a task, the TrEndo recorded four motion analysis parameters (MAPs) voor each hand and the time used to complete the task. For the secondary goal, the time, used on both simulators, was compared. Results: A total of 21 persons completed the course. In both groups, at least five of nine MAPs showed significant improvements between task 1 and 4. Comparing both groups, no significant differences were found after task 4. The subjects, using the intra-operative instructional video, spent significantly less time watching it. Transferability of skills between different simulators was not found. Conclusion:. Compared to intra-operative instructional video’s, the use of pre-operative instructional video’s showed no significant difference in improvement of execution of a laparoscopic task. The intra-operative group used significantly less video time, suggesting intra-operatieve instructional video’s are more efficient at acquiring laparoscopic skills.
