Abstract

Context: While currently in a skin cancer epidemic, skincare is more important than ever in the Netherlands. The incidence of basal cell carcinoma (BCC) has seen a tremendous rise in past decades and it seems to keep rising. At this state, one in six Dutch people will be diagnosed with BCC in their lifetime. A widely used treatment option is Mohs micrographic surgery (MMS), with high cure rates. MMS however, is a time consuming technique. Appropriate indications are important for its efficiency. Objective: Examination of results of MMS-procedures in ZGT Hengelo and analysis of baseline variables as predictors for rounds needed, wound closure, complications and recurrences. Methods: Data about tumor characteristics, rounds needed, closure, complications and recurrences were gathered from 1846 patients that underwent MMS in ZGT Hengelo and were compared to current literature. Baseline variables were analyzed as possible predictors for primary outcomes. Results: Tumors ≤5mm in size were often excised within one round, while larger tumors >20mm in size needed three or more rounds more often. Both tumor location and tumor size had an effect on the chosen wound closure. Tumor location and histopathology had no influence on rounds needed. MMS around the eyes had significantly more complications, as did MMS that were closed by plastic surgeons compared to dermatologists. Histopathology and tumor size did not correlate with complications. Conclusion: The results were largely compliant with current literature from the past 15 years, mainly research based in the Netherlands and found tumor size as a predictor for rounds needed, tumor location for wound closure used and both tumor location and used wound closure for the amount of complications that occurred after MMS.
