Abstract

Introduction For the surgical removal of mandibular third molars, there are two different surgical incision techniques described. The purpose of these incisions techniques is to get clear sight and access to the third molar. With the mesial incision technique, the incision is made mesial of the second molar. The distal envelope incision runs distally of the third molar. The aim of this study is to investigate the effect of two surgical incision techniques, used for the removal of mandibular third molars on the periodontal pocketdepth reduction distal of the second molar. Patients and methods In this study the effect of mandibular third molar surgery on the periodontal pockets depths distal of the second molar is investigated three months after surgical removal. Patients revered to the Oral and Maxillo-­‐facial Surgery unit at Tjongerschans hospital in Heerenveen, were selected for his study. To enhance the reliability of probing of the pocketdepths, this study uses a force-­‐controlled pocket probe (Ash Brodontic©). Results 14 patients were selected fort his study, of which 6 patients (43%) with a mesial and 8 patients (57) with a distal incision technique. Both groups showed a significant pocketdepth reduction of 1,79 mm (p=0.000). There were no significant differences between the mesial and distal incision techniques (p=0.755). The effect of smoking, age and position of the third molar could not be assessed because of the small number of patients. Conclusion This study showed a sigificant difference in probing depth reduction before and after surgical mandibular third molar removal. However this study did not show a significant reduction in probing depth distal of the second molar, between the mesial and distal incision techniques. This study also did not show any significant differences between the effects of smoking and the level of emergence of the mandibular third molar on probing depths after surgical removal of mandibular third molars. The study conceivably has to less power to confirm or to reject these hypotheses. Therefore a prospective cohort study with a larger patient population is recommended.
