Abstract

Introduction: In the Netherlands, the most common forms of treatment of a triggerfinger (TF) are a steroid injection and open surgical release. A steroid injection, a minimal invasive procedure, resolves triggering in approximately 60% of the patients. Open surgical release, on the other hand, has a success rate of 98-100%. but is a more invasive procedure. Despite the high incidence of TF, national and international evidence-based treatment guidelines are unavailable. The purpose of this study is to identify if a steroid injection followed by a triggerfinger release can shorten the functional recovery period. Because of the shift towards patients centered practice in the last decade, emphasize has been laid on evaluating the outcomes on functional activity and patient satisfaction. Methods: Between March 2017 and July 2017, an observational cohort study of patients diagnosed with a triggerfinger was performed. Patients with actively correctable triggering or more severe symptoms, without previous surgical release, were included. Functional recovery and patient satisfaction, measured with the Michigan Hand Outcome questionnaire, were analyzed prior to, and at 2- and 6 weeks postoperatively. Results: A total of 40 patients were eligible for analysis, 24 were treated with a steroid injection followed by surgery and 16 were treated with surgery alone. There was no significant different between the steroid group (77,8%) and the nonsteroid (87,8%) group in functional recovery. In addition, patient satisfaction did not differ between the steroid (71,2%) and nonsteroid group (79,2%). A surprising finding is that 22% of all patients reports that they were not satisfied with their hand function after 6 weeks. Conclusion: A combined treatment of steroid injection followed by open surgery for patients with TF shows no significant improvement in patient-reported functional outcome or satisfaction levels. Further studies with a larger sample size and longer follow-up period are necessary to confirm our preliminary findings.
