Abstract

Background: After ACL injury, most athletes opt for an ACL reconstruction. Despite reconstruction, they show altered movements during walking, jumping and stepping down. It is conceivable that patients consciously avoid loading their involved leg because they are internally focused on their knee. Objectives: The objective of this study is to examine the effect of cognitive motor control in terms of an internal focus and external focus during an easy and common daily task. Methods: Twenty ACL reconstructed patients (10 males, 10 females) and twenty healthy subjects (10 males, 10 females) were included. All subjects were measured once under two different conditions: with and without a virtual reality (VR) environment. Study outcomes were peak ground reaction force (GRF}, knee excursion and maximum internal knee extension torque. The data were statistically analyzed with multi-level models. Results: Peak GRF was significantly higher in the VR environment for both groups. The patients' involved leg and the control subjects' preferred leg had both a significantly lower peak GRF compared with that of their other leg. A significant interaction effect was found between groups and environments. Maximum internal knee extension torque was only significantly higher in the traffic environment for patients. An interaction effect between groups and environment was found. Knee excursion did not show significant differences at all. Conclusions: VR was effective on maximal internal knee extension torque for patients and on peak GRF for both patients and healthy subjects. So, VR has an influence on some movement parameters by distracting the internal focus from the knees and could be used in rehabilitation programs.
