Abstract

Objective. The purpose of this cross-sectional retrospective clinical study is the evaluation of fiber-reinforced composite fixed partial dentures (FRC FPD‟s) made at the Centre for Dentistry and Oral Hygiene (CTM) in Groningen, the Netherlands, to identify its success and survival rates over a 5-year period. The obtained success and survival rates are compared with similar data obtained from literature acquired from PubMed and Cochrane online databases. The results gained are used to clarify and justify the usefulness of this type of dental treatment at CTM. Methods. Electronically available databases at CTM were searched for the term „FRC FPD‟. Every found FRC FPD publication was selected from the scrutinized files. There appeared to be a population of 35 patients (18 females, 17 males) that received 53 directly made FPD`s reinforced by glass fiber. 32 FRC FPD`s (60%) were made in the upper jaw and 21 FRC FPD`s (40%) in the lower jaw. Obtained data were operationalized and analyzed using SPSS software. Kaplan-Meier survival and success rates for these FRC FPD‟s were determined. PubMed and Cochrane online databases were searched for studies up to 2012 revealing survival and success rates of FRC FPD`s. Relevant articles concerning directly made glass fiber-reinforced FPD`s were selected. From these articles relevant data was extracted and compared to the success and survival rates of FRC FPD‟s determined at CTM. Results. The Kaplan-Meier success rate was 35% (SE = 9%) and the survival rate was 28% (SE = 8%) after 5 years in situ. This study revealed significant differences of survival and success rates of FRC FPD`s made in the upper and lower jaws. Success rates of FRC FPD‟s in the upper jaw (53%; SE = 10%) were significantly higher than those placed in the lower jaw (12%; SE = 10%). Survival (28%; SE = 8%) and success (35%; SE = 9%) rates of FRC FPD`s made at CTM were considerably lower compared to data extracted from literature. Conclusions. FRC FPD‟s made at CTM have a relatively low survival expectation. Yet, files regarding FRC FPD‟s at CTM were not complete. Due to this lack of information it was not possible to adequately establish scientific evidence between cause(s) of failure and results observed, which would be valuable in order to establish improvements concerning the FRC FPD‟s made at CTM. Comparison of survival and success rates at CTM with extracted data from literature appears to be questionable, because of the heterogeneity between studies and lack of sufficient evidence concerning directly made glass fiber-reinforced FPD`s. There is an absence of protocol for indication and fabrication of FRC FPD`s. Therefore, there is a need for standardization and international acceptation of protocols for fabrication of directly made glass fiber-reinforced FPD‟s. At this moment, the application of FRC FPD`s at CTM should be described as experimental. Finally, these FPD‟s should not be considered yet as a long-term alternative method to substitute conventional methods for solitary tooth replacement.
