Abstract

Background basal cell carcinoma (BCC) is the most common type of skin cancer with a prevalence of 30% worldwide. Recent literature has shown that after surgical removal of a BCC in the face, concerns about the appearance exist from a patient’s perspective. The aim of this study was to analyze the assessment of the scar through a patient reported outcome measure: the FACE-Q scale. Secondly, potential predictors for assessment of the scars have been evaluated. Methods included in this study are patients treated by excision for a BCC in the face between April 2016 – December 2017. Analysis was carried out on 8 multiple choice questions related to assessment of the scar. The scores were added up and converted to a scale ranging from 0100, with a higher score meaning higher satisfaction. Results a total of 47 males (=44.3%) and 59 females (=55.7%) with a mean of 69 years (±10.9) were included. Median scar satisfaction was 91/100. Multiple predictors for a lower scar satisfaction are lower age (regression coefficient (RC): 0.47 (95% CI: 0.12; 0.82, p = 0.009)) combined with female sex (RC: 0.47 (95% CI: 0.12; 0.82, p = 0.009)) and nonsmokers (RC: 12.22 (95%CI: -0.86; 23.59, p = 0.035)) combined with use of anticoagulant medication (RC: 11.20 (95%CI: 3.27; 19.12, p = 0.006)). Besides age, sex, no smoking and use of anticoagulant medication, a smaller diameter of the tumor (<15mm) is a single predictor for lower scar satisfaction (p = 0.037). Diabetes is a predictor for higher satisfaction (p = 0.045). Conclusion this study shows that patients are satisfied with the appearance of their scar after BCC excision and reconstruction. More explanation regarding the aesthetic result by the plastic surgeon is needed for patients with a lower age (<70 year), female sex, lower age combined with female sex, a small diameter of the tumor (<15mm) and use of anticoagulant medication.
