Abstract

Purpose: The purpose of this study was to examine the effects of exercising the non-operated limb for 5 weeks after anterior cruciate ligament (ACL) reconstruction. Participants: Four subjects awaiting ACL reconstruction surgery participated in the pilot study. Methods: This study was a pilot for a large single-blind randomized clinical trial. Cross-education group and standard care group were tested on a dynamometer 1 week before surgery and 5 weeks after surgery. The main outcome measures were maximal voluntary strength, force inaccuracy and steadiness during eccentric, concentric and isometric contractions. Secondary outcome measures were Cincinnati Knee Score and Visual Analogue Scales (VAS). Results: The ratio of maximal isometric voluntary strength in the injured versus non-injured limb decreased for both standard care (- 49.9%) and cross-education group (-48.9%). Almost all inaccuracy force ratios of the standard care group increased after surgery, which mean decreased accuracy in the operated leg relative to the non-operated leg. Almost all the inaccuracy force ratios of the cross-education group decreased, which represent greater performance in the operated leg relative to the non-operated leg. Force unsteadiness increased for the standard care group and decreased in the cross-education group for almost all force ratios. Cincinnati Knee Scores increased in the cross-education group 9.7% and decreased 33.9o/o for the standard care group. VAS scores increased 20.8o/o respectively 85.5% for the standard care and cross-education group. Conclusion: The findings of improved force control after 5 weeks of additional cross-education exercises may have potential implication for the use of cross-education in the rehabilitation of patients recovering from an ACL reconstruction.
