Abstract

Functional tremor is a common type of tremor and part of a wide spectrum of functional movement disorders. Patients with functional tremor report severe impairment. However, these subjective symptom reports have not yet been compared with the objective symptom severity. The aim of the study is to investigate on an individual basis if there is a correlation between subjective and objective tremor levels. We also compare the functional group with the neurological group to investigate a possible difference. In this study participated 15 patients with tremor, 3 functional and 12 neurological. We assessed the tremor both objectively and subjectively for a period of 30 days. The objective assessment by means of an actigraph during the day, subjective by means of multiple online journals 5 times a day. With the program Autovar the data is analyzed for a possible vector auto regression models. In two patients there was a correlation found between the subjective and the objective measurements, however in most patients there was no correlation. The data does show clear fluctuation in the tremor scores, the amount of tremor severity and complaints varies. When comparing the two groups, the functional tremor patients report a significantly higher subjective symptom level compared to patients with neurological tremor. Yet, the objective level was significantly lower in functional tremor patients. The data suggests that the subjective symptom report of a patient does not necessarily reflect the severity of the tremor, this is especially important in the evaluation of tremor in the clinical practice.
