Abstract

Introduction: Little is known about the influence of socioeconomic status on food allergy prevalence. Studies that address the association of socioeconomic status and food allergy indicate that patients with low socioeconomic status are less likely to have food allergies, but this has not been confirmed in all studies. All of these studies have a major limitation in that they are not based on objective oral food challenge (OFC) data. This study aims to research this clinical observation using OFC data. Methods: Data on OFCs was collected from March 2008 to April 2015 in Tufts Medical Center and Massachusetts General Hospital. Socioeconomic status was determined by annual median household-income, based on zip code. Annual household-income was categorized in ≤$50,000 or >$50,000. Results: Data on 1252 OFCs performed in 831 patients was retrospectively analyzed. Average age was 9.7 years (range 0.7-79.2). Majority was male (59.9%) and white (81.2%). We found that patients from families with lower household-income are more likely to have a negative oral challenge with an odds ratio of 2.3 (95% CI 1.23-4.32, p = 0.009). Conclusion: In this study, we demonstrated patients with low socioeconomic status have negative OFC outcomes more often, thus ruling out allergy. The reasoning behind this finding and the relevance of this finding need to be further elucidated.
