Abstract

Study objectives: The Amsterdam PrEP (AMPrEP) project is a demonstration project focused on pre-exposure prophylaxis (PrEP) as HIV prevention strategy for men who have sex with men (MSM). In some MSM, constant fear of HIV has profoundly affected the capacity to enjoy sex. The sexual satisfaction study is part of the AMPrEP project and aims to gain insight in sexual satisfaction of HIV-negative MSM before and after starting PrEP. Design: Longitudinal study among 297 HIV-negative participants of the AMPrEP project. Methods: Socio-demographic, sexual behaviour, HIV risk-behaviour and perception and health-related characteristics were described. A paired sample t-test was used to investigate the difference in sexual satisfaction between baseline and month 1. Multivariable linear regression was used to identify factors associated with sexual satisfaction. Results: Sexual satisfaction at baseline was 43.9 (SD 8.18) on a scale ranging from 12 to 60. In the multivariable analyses, sexual compulsivity (β -0.22; CI -0.39 to -0.05) and mental health (β 5.38; CI 2.95 to 7.81) were significantly associated with sexual satisfaction at baseline. The sexual satisfaction score significantly increased after using PrEP for one month with a mean of 46.2 (SD 7.45; P value <0.001). In the multivariable analyses, none of the variables were significantly associated with the change in sexual satisfaction between baseline and the first month after starting PrEP. Conclusion: Our findings indicated that good mental health is associated with a higher score on sexual satisfaction. Findings also show that participants with symptoms of sexual compulsivity have a lower sexual satisfaction. MSM using PrEP for one month have a small, but significant, increase in sexual satisfaction. We cannot exclude that the increase in sexual satisfaction after the first month of PrEP is temporary and therefore further analysis with longer follow-up is necessary to investigate the influence of PrEP on sexual satisfaction.
