Abstract

Subject: This study evaluates the care for children with Down’s syndrome (DS). It includes research on health care utilization and health care needs of children with DS, as well as research on the needs and satisfaction concerning the care for children with DS from a parental perspective. The obtained information will be used to underpin the content of the polyclinic. Methods: All children with DS born after 01/10/1990 and who have visited a pediatrician at a hospital in Twente in the past 10 years, were included in the retrospective study. Their conditions, the outpatient appointments, additional research, hospitalization and length of stay and surgeries of the children (n=201) were put in a database. Out of all patients included in the retrospective part of the study we asked the parents of children born in Twente after 1992 to complete a survey. The survey identifies visits to caregivers and participation in the healthcare screening program for children with DS. Other subjects were participation in a development program, breastfeeding and the need for a specialty clinic for children with DS in Twente. There were 119 (67%) respondents. Results: Congenital heart disease (57.2%), problems of the gastro-intestinal tract, respiratory infections and skin disorders are common problems in children with DS. Children with DS visit more often a medical specialist (p = 0.000), are more often hospitalized, have longer hospital stays (p = 0.000) and undergo more surgeries (p = 0.000) than an average Dutch child. Children with DS in Twente visit less often a specialty clinic (p = 0.00) than children with DS in the Netherlands. Out of all parents, 58.5% will visit and 32.7% might visit a future specialty clinic in Twente. Children with DS in Twente complete up to 60% of the 24 points of the Dutch screening program for children with DS. Children with DS in Twente participate less frequently (p = 0.00) in the Early Intervention program than children with DS in the Netherlands. Children with DS get less frequently breastfeeding (p = 0.00 - p = 0.011) than the average Dutch child. Conclusion: Among parents in Twente there is a need for a specialty clinic for their child with DS. Children with DS do have a higher consumption of healthcare. A lot of children with DS in Twente don’t get the recommended care they might need.
