Abstract

Objective: The objective of this study is to investigate the extent to which oral care plays a role in home care for frail elderly and what the knowledge is of the nursing staff on this subject. Research question: What is the level of knowledge about oral hygiene among nursing staff working in home care for frail elderly and what is the influence of the degree of training or familiarity with the content of an oral care protocol? Materials and methods: Two online surveys were distributed to the nursing staff and the management of umcGroningen thuis. In both surveys are organizational questions as well as the attitude and perception towards oral care covered. In the survey for the nursing staff is further questioned about educational background, questions at client level and a significant part consists of knowledge questions. The results were analyzed using descriptive statistics, the Mann-Whitney-U-test and Spearman correlation test. Results: 32% of the nursing staff (N=25) and 66% of the management (N=2 ) participated in this study. The study shows that (especially in the morning and/or evening) oral care is part of the daily care to frail elderly. UmcGroningen thuis gets an average mark on this of a six. The oral care knowledge remains defaulted on some essential topics; cleaning method and/or the storage medium of structures other than the natural set of teeth (e.g., the (partial) denture / implants), and the maximum recommended daily sweet moments. Very few people of the nursing staff are familiar with the contents of the available oral care protocol and internal cooperation with a dental care professional hasn’t been discussed so far. Where only a small part of the nursing staff has followed (external) additional training about oral care, the majority got well paid attention about it in the basic education. No significant difference was found in height of the achieved knowledge score and the degree of training/familiarity with the content of an oral care protocol. The correlation between the graduation year and the knowledge score was statistically significant. As one graduated longer ago a higher knowledge score was observed. Discussion: Larger scale studies are required to provide a more representative conception of the situation in other regions / organizations and to demonstrate any causal links. Conclusion: Oral care plays a role in (the care for frail elderly of) umcGroningen thuis, but there is an occasion for improvement in the organization and knowledge on this subject.
