Abstract

Titanium internal fixators have been regarded as the gold standard in the maxillofacial surgery for a long time. These fixation systems are considered to be easy in handling and effective in therapy. However, there are also disadvantages regarding these systems. Second interventions for removal of the fixators and restrictive cranial growth are just a few examples of the disadvantages related to the use of these systems. Biodegradable internal fixators have increasingly served as an alternative for these titanium systems. These biodegradable plates and screws are recommended for use in the maxillofacial region because of their intrinsic properties and ability to be biocompatible and fully biodegradable. The purpose of this study is to systematically review the available literature to determine the evidence supporting full degradation and resorption of these biodegradable polymers up to the level of electron microscopy (EM) with a special interest in four biodegradable systems used in maxillofacial surgery: ResorbX/SonicWeld RX®, Biosorb FX®, Inion® en Lactosorb®. A highly sensitive search of the MEDLINE database was conducted without restrictions regarding date of publication (-2011 ), type of study or type and brand of biodegradable implant. Eligible studies were independently evaluated by two assessors. The selection procedure resulted in 88 relevant articles which have been methodologically assessed after inclusion. Numerous studies have observed full degradation or resorption up to the level of CT, MRI and light microscopy. However, there does not appear to be any evidence supporting full degradation or resorption up to the level of electron microscopy in the current literature. Therefore, definite conclusions regarding the time-span of full resorption (up to EM level) can not be drawn. There is a strong need for long-term degradation studies which evaluate the full resorption up to the level of electron microscopy. Keywords: Biodegradable, Osteosynthesis, Resorption
