Abstract
Summary Introduction In patients with persistent pain in the orofacial region, changes in the nervous system occur. In addition, psychosocial factors become more prominent. To diagnose pain, different classification systems have been developed. In these classifications, the pain conditions are usually described in terms of symptomatology. When the clinical presentation of symptoms becomes less clear, because of changes related to chronocity, classification will be more difficult. This will hamper the diagnostic process and subsequent treatment of these patients. The aim of this paper was to systematically assess the literature of the last five years concerning the quality of clinical diagnostic tests performed in patients with persistent orofacial pain, subsequently, to assess the clinical feasibility of these tests. Methods For this systematic review a search was conducted with specific search terms for clinical studies in orofacial pain. Of these articles, the titles and abstracts were screened for relevance. In order to obtain qualitatively adequate studies, eventually 13 studies were assessed using the Quality Assessment of Diagnostic Accuracy Studies list (QUADAS). This checklist consists of 14 items that refer to internal validity. Studies scoring “yes” in more than half of the questions were included for further appraisal. Results The search retrieved 200 articles, of these articles 12 were found to be of sufficient methodological quality. Of the included studies, seven investigated the diagnostic value of magnetic resonance imaging and angiography (MRI/MRA) for vascular compression in trigeminal neuralgia. Three studies reported de diagnostic value of ultrasonographic imaging and MRI in diagnosing internal derangement. The others concerned research into the validity of the different items the diagnostic tool, Research Diagnostic Criteria for Temporomandibular Disorders, consist of and research into the sensitivity of computed axiography and MRI in the diagnosis of articular disc displacement and the presence of degenerative joint disease. Discussion When literature concerning the diagnostic tests in orofacial pain conditions is reviewed and qualitatively appraised, only a minority remains, which concerns mainly well defined orofacial pain. Conclusion This review demonstrates that when studies on diagnostic tests in orofacial pain conditions are reviewed, only a minority remains. Moreover, these studies predominantly aim at well-defined orofacial pain types, which are relatively well described and relatively easy to diagnose. There remains a large need for diagnostic methods aiming at (more) complex pain conditions, such as chronic orofacial pain
