Abstract

Background: There exist many different instruments to assess the quality of learning environments in postgraduate specialist training. However, these instruments consist of relatively long questionnaires which increases the likelihood of fairly low response rates. It also appears that a screening instrument to measure the quality of the education should cover three domains: 1) personal development/goal direction, 2) relationships and 3) the organization/regulation of training. Aim: The aim of this study was to develop a brief and applicable screening instrument (called ‘ postgraduate training thermometer’) to assess the quality of learning environments in postgraduate specialist training, suitable for annual use, and covering the three domains of the learning environment. Methods: The items of ten (inter-) nationally available questionnaires were used as a basis for the development of the ‘PTT’. These items were presented to a panel of experts who completed a Delphi-procedure. In each round of this procedure, the experts selected the most relevant items and completely covered the three domains. In addition, junior doctors were interviewed to explore their views on the ’PTT’ and to identify factors that increase response. Results: Seven experts completed the Delphi-procedure. After three rounds, agreement was reached upon the items that need to be included in the ‘PTT’. In the final round of the procedure, a few items were added as a result of the 26 interviews which were held with the junior doctors. The results of the interviews also showed that the following factors may increase response: display the progress when filling in the screening instrument, timing of the distribution of the questionnaires (not on Friday / Friday afternoon) and provide (anonymous) feedback of the results. Conclusion: Based upon the results of the Delphi-procedure and the interviews with the junior doctors, a concept of the ‘PTT’ was created, consisting of 43 items distributed across the three domains
