Abstract

Objective The aim was to investigate the prevalence and 4-year incidence of extra-articular manifestations (EAMs, acute anterior uveitis (AAU), inflammatory bowel disease (IBD) and psoriasis) in the Groningen Leeuwarden Axial Spondyloartritis (GLAS) cohort. And to identify possible associations between clinical parameters and EAMs. Methods To investigate the prevalence and incidence of EAMs baseline and follow-up data from the GLAS cohort were used and the medical charts were checked. the logistic regression and Cox regression were used to investigate associations between clinical parameters and EAMs at baseline and during follow-up. Results In this study with 421 included patients (mean age 43.4 year (sd. 12.5), 272 (64.6%) men, symptom duration 16.0 year (1-59) and 320 (77.5%) HLAB27 positive) prevalences of 24.8% for AAU, 9.5% for IBD and 4.3% for psoriasis were found. In 362 patients with 4 year follow-up data the incidence of AAU was 4.8%, 2.5% for IBD and 2.0% for psoriasis. At baseline AAU was more frequent observed by patients with a HLAB27 positive status, longer symptom duration and non-smokers. IBD was associated with a lower quality of life. Psoriasis was associated with longer symptom duration, HLAB27 negative status, and more spinal radiographic damage. During follow-up patients with a history of AAU did more frequent develop a recurrent episode of AAU, but starting anti TNF reduces the change on developing a recurrent episode of AAU. Conclusion In this Axial SpA population high prevalences are found, the highest for AAU. Incidences rates are much lower, probably caused by the long symptom duration in this population through which EAMs already developed during the course of the disease. EAMs are associated with different parameters and the development of AAU is more frequent observed in patients with an history of AAU. Starting anti TNF treatment reduced the frequency of recurrent AAUs.
