Abstract

OBJECTIVE: Endoscopic sinus surgery (ESS) is the treatment of choice for chronic rhinosinusitis (CRS) when pharmacological treatment does not sufficiently resolve symptoms. Health related quality of life (HRQoL) has become an important outcome measure in the evaluation of treatment success. The aim of this study was to evaluate the effect of ESS on HRQoL in CRS patients using the Endoscopic Endonasal Sinus and Skull Base Surgery Questionnaire (EES-Q), and to determine relevant patient and disease related characteristics influencing the effect of EES on HRQoL. METHODS: We enrolled 75 patients diagnosed with CRS treated with ESS in this prospective cohort study in the UMCG, a tertiary referral centre, between November 2016 and August 2018. The EES-Q was used to compare HRQoL pre- and postoperatively, with follow-up two weeks, three months and one year postoperatively. Various patient and disease specific factors were evaluated in multivariate logistic regression analysis to determine associating factors for postoperative HRQoL. RESULTS: The mean age of the patients was 50.1 ± 14.6 years, and 41 (54.7%) were males. In our study sample 66.7% of patients underwent revision surgery. EES-Q scores preoperatively ranged from 1.20-4.03 and postoperatively from 1.00-3.79. Clinically relevant and statistical significant improvement in total EES-Q scores was shown three months (0.52, p < 0.05) and one year postoperatively (0.57, p < 0.001). In univariate regression analysis no influencing patient or disease specific factors showed a significant association, however preoperative EES-Q scores showed an association (p < 0.05) in a multivariate model including hypertension and the use of postoperative systemic corticosteroids. CONCLUSION: HRQoL significantly improves after ESS in CRS patients three months following surgery and seems persistent. Further research is needed to define predictive factors affecting HRQoL outcomes after ESS, in order to optimize patient counselling.
