Abstract

Each year over 14,000 women in the Netherlands are diagnosed with breast cancer17. Due to early detection and still improving treatment results, breast cancer survivors are increasing by number. Therefore increasing numbers of females are at risk for long-term health problems. Radiation therapy is part of breast conserving therapy and is thought to be of influence on developing cardiac disease later in life2,4-8, 11, 12, 16, 23, 30, 34 . Recent study by Darby et al. shows an excess risk of cardiac disease in breast cancer patients. They developed a model that showed an excess risk of 7.4% per Gray mean heart dose 12. This study was performed on patients treated over several years from Denmark and Sweden. The radiation techniques were old-fashioned and heart dosage was estimated, which was reason enough for the department of Radiation Oncology of the UMCG to validate this model with actual heart doses and is therefore the main subject of this paper. We found out that patients over 60, with at least one Cardiac Risk Factor and a high Mean Heart Dose, were significantly at risk of developing Ischemic Heart Disease. Although we found a higher number of Acute Coronary Events than was initially predicted by the use of the Darby model, this model can be of great value in the decision making process of adjuvant therapy in Breast Cancer Patients. The Mean Heart dose is a significant factor, that, with the help of CT-planning might be decisive in whether to use the regular irradiation technique or the more advanced and less cardiac involving Proton irradiation.
