Abstract
Introduction- Aim of this research was to identify neonatal EEG (aEEG, ASI) and MRI markers for later development of electrical status epilepticus in sleep (ESES) and neuropsychological deficits after neonatal thalamic injury. Methods- This retrospective longitudinal cohort study included patients with neonatal thalamic injury. Data collection: EEG characteristics; aEEG background pattern, epileptic activity, ASI 2.5 min, childhood sleep-EEG analysis and spike-wave index (SWI) calculation. MRIcharacteristics; thalamic injury location, overall abnormalities at neonatal age, ~4 months and childhood. Statistical analysis was conducted, p-values <0.05 were considered significant. Results- 26 patients with neonatal thalamic injury were included, 50% developed ESES, 30.8% ESES-spectrum disorder and 19.2% no SIEA within follow-up. aEEG background pattern was abnormal in 80%, epileptic activity on aEEG was present in 86.7%, mean ASI 2.5 min was 2.08 (under normal cutoff ~3.6). On sleep-EEG mean SWI was 73.54. Percentages for thalamic lesion locations varied between time periods. Conclusion- No clear early MRI or EEG predictors were identified for the severity of sleep-related epileptic abnormalities in this population. Thalamic lesion location (bilateral injury, anterior nucleus involvement) is related to neurodevelopmental and total IQ / DQ decline. The severity of epileptic activity in ESES patients corresponds to severity of neurodevelopmental deterioration.
