Abstract

Objective: It is important to screen the periodontal tissues. Not only to determine whiter the tissues are inflamed or not and possibly need treatment, but also because periodontitis could have an influence on the overall health of the patient. The method that is used in the Netherlands at the moment for this screening is called the Dutch Periodontal Screenings Index (DPSD. This test can only be applied by a dentist or a dental hygienist. To make the screening for periodontitis more accessible and also executable by (para)medics there is a test developed which is simple and non invasive so other medical staff could also applied it. If the test is positive than they could refer the patients to the dentist for further investigation. Methods: To examine the effectiveness of the Periodontal Screening trough Saliva (PSS) test 67 patients who were getting dental treatment at the Centrum voor Tandheelkunde en Mondzorgkunde (CTM) in Groningen where examined They underwent the DPSI as well as de PSS test. In this population there was a wide spread in terms of age, degree of periodontal inflammation and medical status. The PSS test strips where visually analysed as well analysed by the Urysis 1100. From every test subject was noted whether they smoked, had a wound in their mouth, had a medical abnormality/disease and/or used medication. Results: There is a strong and significant relationship between the DPSI and the amount of erythrocyte measured by the PSS test with a sensitivity of 92.5%, a specificity of 77.8% and a validity of 86.6%. There was no strong relationship between the amount of leukocyte measured by the PSS test and the DPSI. They also didn't have a predictable value for the grade of periodontal health. Further analysis showed that the Urysis 1100 had a strong internal consistency with the Visual analysis. Analysis of PSS test using the Urysis 1100 turned out to have a slightly stronger significant relationship with the DPSI than the visual analysis. Conclusions: There is a strong relationship between the amounts of erythrocytes measured by the PSS test, not for the leukocytes. There is a good internal consistency between the visual analysis and the analysis trough the Urysis 1100. A false positive or false negative test result isn't seen that much. The previous defined patient characteristics (smoking, having a wound in their mouth, having a medical abnormality/disease and/or using medication) could have a negative effect on the sensitivity, specificity and validity of the PSS test, but this needs further research to be able to draw conclusions about this aspect. Overall, you could say that the PSS test could be applicable as a screening instrument by paramedics to determine the grade of inflammation of the periodontal tissue's as a contributing factor for other health issues.
