Abstract

Introduction: Dental emergencies aren’t limited to office hours. To help patients in the Netherlands there is an out of office hours emergency service (from now on ‘the emergency service’). To guide dentists on how to arrange this service the KNMT has developed a guideline called “Opvang tandheelkundige spoedgevallen buiten praktijkuren-2012”. The main purpose of this study is to evaluate the guideline on its use and content. Furthermore, this study investigates how the emergency service is organised throughout the Netherlands. Method: The use of the guideline has been evaluated using the Lanting method. In this method the five most frequently mentioned core-elements have been included (these are the five elements that the developers believe are most important to record in the guideline). Inclusion of these core-elements is deemed acceptable in this paper if 80% of the groups of dentists incorporates them. Furthermore, at least 90% of the group leaders should be aware of the existence of the guideline. Finally, a total score is used to judge the use of the guideline. For every element of the guideline that the group complies with one point is given. Both use, content and organization have been questioned in a questionnaire which has been sent to 167 group leaders by the KNMT. Results 89 of the group leaders responded (53%) Use: 85% of the group leaders know about the existence of the guideline. 58% of the groups have manuals containing a clear definition of an emergency. 64% of the groups have disclosed which contributors are part of the emergency service and how much times they are working. 90% have demarcated the area of their service. 68% of the dentists can be reached by phone at all times. 43% of the groups have made appointments of rebuke after inadequate performance of service. 19% of the groups follow the guideline closely. Content: 72% of the group leaders declare the mapping of the information of the guidelines as good. 74% judged that the clarity of the recommendations where good. 58% thought that the completeness of the recommendations where good and 60% thought the usability of the recommendations where good. Organisation: The average group has a combined weekend- and evening service, has 45 dentists, and takes care of 84832 patients. Patients arrive at the consultation hour mostly after direct contact with the dentist. A patient can obtain the phone number of the emergency service in several ways. When the groups use triage, the patients will mostly have contact with an administrative assistant, a dental assistant, or a nurse. In most cases there is a protocol provided for these contributors, and in half of the cases there is a person they can call when they are in doubt. The group leaders opine the triage as good in most cases. After-treatment reporting is provided to the own dentist. In most cases this happens in writing. The majority of the groups have regulations, the content of which differs per group. Also, the requirements to participate in the emergency service differ per group. Almost every group has a coordinator and in almost half of the a vice-coordinator. The activities of these persons differ per group. Conclusion: Use The target values of the Lanting method are not realised, with the exception of the core-element “description of the area”. Both the knowledge of, and the compliance with the guideline should be increased to realise these values. However, it can be debated whether this is necessary as this research finds that the group leaders barely observe bottlenecks with after hour treatments. Content The aspects of the guideline that were researched in this study were labelled as being ‘good’ in 60-74% of the cases. It can be determined that the content of the guideline has been reviewed as moderate by the group leaders. Organisation The organisation differs between the groups. The combined weekend and evening emergency service is most frequently used, and Triage is a more and more occurring phenomenon.
