Abstract

To give the best possible care to the increasing number of patients with asthma and/or COPD, an Asthma COPD (AC) service in the Northern part of the Netherlands has been developed. This A/C-service supports general practitioners in the diagnosing and management of patients with asthma and/or COPD. In this thesis, I show the baseline respiratory morbidity in primary care adult patients as presented to the A/C-service for the first time and I compare these results with international populations. Patients who have either symptoms and/or use inhaled medication are referred to the A/C-service. Patients complete questionnaires (History, ACQ and CCQ), a lung function assistant performs spirometry and feeds data to the electronic data file accessible via internet. Based on these data the local pulmonologists give the diagnosis and advice about treatment to the general practitioner (GP). In the end, the GP is responsible for the management of their asthma/ COPD patients. Data were collected from 2007 till 2012. A total of 9.681 patients have entered this project of which 44,5% are males. Mean age was 54,1 year. Asthma was diagnosed in 44%, COPD in 19% and mixed asthma and COPD in 7% of the patients. GOLDI and II was diagnosed in 83% of the COPD patients. Unstable COPD, defined as a total CCQ-score >1 was found in 64,1% of the COPD-patients of whom 33% were at risk for exacerbation thus resulting in the following risk categories according to GOLD guidelines version 2012: A 28%, B 39%, C 8% and D 25% D. Uncontrolled asthma, defined as an ACQ-score >1,5 was diagnosed in 40% of the asthma patients and 34% had well controlled asthma, defined as an ACQ-score <0,75. Current or ex-smokers tended to have more often uncontrolled asthma. These data led to the conclusion that in primary care asthma is far more prevalent than COPD. The results show that there is, with help from the A/C-service, sufficient room for improvement in primary care asthma and COPD management.
