Abstract

INTRODUCTION ̶ The incidence of running related injuries (RRI), especially from the knee, is high among recreational runners. Looking from a biomechanical perspective, excessive hip adduction possible plays an important role in the development of RRI. OBJECTIVES ̶ The purpose of this study is to analyze the effect of internal and external feedback on hip adduction and other biomechanical factors during the running gait of recreational runners. DESIGN ̶ Pilot study. METHODS ̶ 28 participants, from whom 7 runners with a knee related RRI and 21 runners without RRI were included in this study. Randomization (1:1) determined the type of feedback: internal feedback (verbal instructions) or external feedback (increasing step rate with 7,5%). Using 3DGait analyze, subjects were measured at their original gait, their gait with feedback and their gait after a month follow-up. The primary outcome was the peek hip adduction angle, secondary outcomes were the knee abduction angle, pelvic drop and pain scores of the knee (α <0,05). RESULTS ̶ There were no significant differences by internal feedback (T0:7,78⁰, T1:8,31⁰, T2:10,62⁰) and external feedback (T0:8,30⁰, T1:7,69⁰, T2:8,78⁰) for hip adduction analyzed per protocol. Subanalyse resulted in a significant reduce of the peek hip adduction angle (-2,62⁰) (p=0,012) after giving external feedback to participants with a relatively high baseline peek hip adduction angle (T0:9,97⁰, T1:10,97⁰, T2:7,17⁰). CONCLUSION ̶ External feedback can reduce peek hip adduction in the running gait of recreational runners with a relatively high baseline peek hip adduction and can be a useful treatment for knee related RRI and for prevention of injuries in subjects with no history of RRI.
