Abstract

Introduction: In order to limit rising healthcare costs, general practitioners (GP) increasingly make use of digital consultations to seek counsel with specialists. This is called teleconsultation, and the goal is rapidly acquiring specialist advice and prevent referrals. Teledermatology in particular has proven to be cost-effective. However, tele-ENT (ear nose throat) is not yet routinely applied in the Netherlands. This study examines the cost-effectiveness of tele-ENT and may be a set-up for a large-scale follow-up study. Methods: In this prospective, observational study, 10 patients were enrolled and 11 tele-ENT consultations took place. These patients visited the GP in the period of October 2017 to April 2018 with ear complaints or an abnormal eardrum, and of which the GP had doubts about the policy. In the intervention group the history taking and photograph of the eardrum were sent to the otolaryngologist. In addition, the doctor noted what the policy would have been without tele-ENT (self-handling, telephone consultation or referral), which in turn formed the control group. The outcomes were costs, patient satisfaction and quality of the teleconsultation. Results: In the control group, two situations were compared: telephone consultation would always (1) or never (2) result in a referral. Tele-ENT was significantly cheaper in both situations, with a mean difference to the control group of €50 and €130 per patient respectively. The mean rating of tele-ENT given by the patients was of 7.6. The photograph was properly interpretable in 45% of the tele-ENT consultations, 73% of the teleconsultations were perceived as useful. Conclusion: The results of this study are promising: the use of tele-ENT was cost-effective and provided good patient satisfaction. Further studies with a larger patient populations are needed to further investigate these results.
