Abstract

Introduction: Adherence to the LDL target value (≤1.8 mmol/L) in the secondary prevention of cardiovascular disease (CVD) established by the ESC seems to be difficult in daily practice. This leads to discussion about the applicability of this target value, especially in primary care. The aim of this research is to determine to what extent the target value in primary care is maintained and their associated determinants. Methods and results: A retrospective cohort study was performed in post-myocardial infarction patients after discharge from a prevention program at the Deventer hospital. The secondary and primary care data of 126 patients <75 years, who achieved the target value upon discharge, were collected at discharge and one year of follow-up. After follow-up, 107 patients (84.9%) preserved the target value. A linear regression showed that the LDL-concentration at discharge was positively associated with the concentration after follow-up (β=0.637 95% CI 0.415- 0.858). Interestingly, 90 (71.4%) patients remained under control of the cardiologist after discharge from the prevention program, but this was not associated with maintaining the target value (OR=0.874 95% CI=0.294-2.425 p=0.753). Conclusion: The LDL target value in secondary prevention of CVD appears to be maintained in almost 85% of the post-myocardial infarction patients one year after discharge from a prevention program at the Deventer hospital. However, there may be room for improvement through individualized education and guidance.
