Abstract

Background: Overweight is an expanding problem in the western society. The intra uterine environment may play an important role in the development of overweight and several studies indicate that maternal smoking during pregnancy is associated with increased BMI in offspring. Objectives: The aim of this study was to asses at what age the association became apparent of maternal smoking during pregnancy and altered growth in offspring. Methods: Prospective data of 1153 children, enrolled in the GECKO Drenthe Cohort, were analyzed to investigate if maternal smoking during pregnancy was associated with changes in weight-for-age, length-for-age and BMI-for-age z-scores at the age of 1, 6, 11, 18, 24, 36 and 45 months. These associations were analyzed using multivariate linear regression, adjusted for birth weight, birth length, prematurity, breastfeeding, parental BMI and maternal education level. Additionally, logistic regression was performed to analyze the risk of childhood overweight. Results: Compared with children of mothers who did not smoke during pregnancy, children of mothers who smoked during pregnancy were heavier from the age of 6 to 36 months (β 0.291, 95% CI 0.097, 0.485) and had a higher BMI from the age of 6 to 45 months (β 0.236 95% CI 0.045, 0.427). Logistic regression showed that from the age of 24 to 45 months children of smoking mothers were at increased risk of overweight (OR 2.5 95% CI 1.24, 5.14). Conclusion: The association between maternal smoking during pregnancy and an increase in weight and BMI in their offspring is already present from the age of 6 months, which emphasizes the importance of very early prevention interventions in the battle against overweight and of increasing the awareness of the side effects of smoking among future mothers.
