Abstract
Since the seventies, the satisfaction of the people with the health system declined. The increased demand for health care resulted in rising costs and longer waiting lists, what made the health care system, characterized by government regulation, came under pressure. After three decades, all political parties in the Netherland's agreed that the health care system was in need of renovation. Also the direction of that renovation had a large parliamentary majority: regulation from above should gradually make place for controlled competition1 • Since 2006, legislation was introduced to promote market forces in healthcare. The shift from supply-driven to demand-driven care had to increase efficiency. There are plans to replace the current maximum rates in dental care with free rates. This should improve the quality, accessibility and affordability of dental care. The question is whether these positive effects may be expected. Partly because of the (potential) shortages of certain medical specialists, the limited enrollment for medical studies are also a subject of debate. Despite the debate about the structure of the healthcare system, the attitudes of the actors in the ( dental)care are unknown. With a web survey has been attempted to answer the following question: What is the attitude of master dental students from Groningen on developments in dental care on the regulation of rates and the capacity of medicaVdental studies? Through literature on market forces in health care and dental care in the Netherlands, a theoretical framework has been made. This describes; (changes in) the health care system, (changes in) the dental care market, economic theories, (future) views on competition in the (dental) care and recent developments. To determine the attitudes, 29 propositions have been formulated. The dental students had to answer, on a 5 point Likert scale, into what extent they (dis )agreed with the statements. The web survey was delivered by e-mail. After 2 weeks 67 of the 134 dental students from Groningen had returned the web survey. The resulting data has been analyzed with the statistical program SPSS. The factor analysis showed that several statements didn't correlate with the assumed attitude. The assumed attitude on market forces in dental care consisted of two components: considering the consequences for patients and for dentists. Also a fifth attitude was "discovered" ; the attitude of the students on the scale of individuality-solidarity. The results show that the master dental students from Groningen answer predominantly conservative to the statements, implying that they have a rather negative attitude toward possible changes in dental care. Between different individual characteristics, such as; gender, political preference and the educational level of their parents, one attitude was significantly different. The attitude towards limited enrollment ofmedicaVdental studies was more liberal in the political left oriented students than the more political right oriented students. Although, it is uncertain how well the respondents represent the overall study population. The aim of this paper is to contribute the debate about market forces in ( dental)care.
