Objects: The aim of this study is to evaluate the effect of the interval between cessation of anti-platelet therapy and surgery, measured by the recurrence rate Methods: In this retrospective cohort study, 151 patients, who underwent surgery for a chronic subdural hematoma between January 1st 2004 and December 31st 2018, in the University Medical Centre Groningen, met our inclusion criteria. Variables were extracted from the electronic patient records. After descriptive statistics, all patients using anti-platelet agents were divided into two groups. Patients were assigned to group A if they used antiplatelet medication and had stopped five days or more, prior to surgery. Patients were assigned to group B if they were taking anti-platelet medication but did not stop within five days of surgery. Results: From 9 patients that met our inclusion criteria, the date that they stopped their antiplatelet agents was unknown, so 40 patients were in group A and 102 patients in group B. Group A and B were compared through the Fisher’s exact-test, Pearson’s chi-square test and Wilcoxon-Mann-Whitney-test, which showed no significant differences, except for hematoma volume. A multivariate analysis with the recurrence rate as dependent variable was considered not significant, despite adding different cofounders. Conclusion: Cessation of antiplatelet therapy for less than 5 days prior to surgery for cSDH, does not affect the recurrence rate.
