Abstract

Introduction: Lung transplantation is in various lung diseases the last possible treatment. However, it is limited by the number of available donor lungs. Moreover, once donor lungs are accepted and transplanted, the mortality rate is still 5-15% because of several factors including primary graft dysfunction (PGD). PGD is the result of ischemia and reperfusion injury and apoptosis is a part of that. The use of lungs from non-heart beating donors in addition to heart beating donors is an opportunity to reduce the shortage of qualified lung donors. Good results of non-heart beating lung transplants raise new questions with respect to the role of apoptosis and the possible relation between apoptosis and PGD. Objectives: The objective of this study was to determine apoptosis in heart beating versus non-heart beating donor lungs. Furthermore, the relation between apoptosis and post-transplant lung function was evaluated. Methods: From 15 patients receiving donor lungs, blood samples were obtained during and after transplantation and ccCK-18, a marker for apoptosis, was determined in the plasma. Results: This study showed that the level of apoptosis is higher in donor lungs from heart beating donors. The relevance of this fact was not fully clear. Level of apoptosis did not correlate with the grade of PGD and therefore apoptosis does not seem to be a good predictor of PGD. Remarkably, higher levels of apoptosis were associated with better oxygen exchange in the lungs. The conclusion is that there are differences between the two types of donor lungs in the way they suffer from ischemia and reperfusion injury when looking at apoptosis.
