Abstract

Background: The pathophysiology of bipolair disorder is still partially unknown. To understand the white-matter theory of the bipolair disorder the fractional anisotropy of euthymic bipolair I patients compared to healthy controls was investigated. Methods: Fractional Anisotropy(FA) values of euthymic bipolair I patients (N=21) were measured with Diffusion Tension Imaging (DTI) and Tract Based Spatial Statistics (TBSS) and compared using SPSS to healthy controls (N=15). A correlation between on the one hand FA-values and on the other hand psychopharmaceuticals, IDS-scores, age, disease duration and sexe. Results: A significant higher mean FA-value was found in the group of euthymic bipolair I patients in comparison to healthy controls (p=0,0015, α=0,05). Additionally a negative corelation (-0,271) between FA-values and age was found (Pearson’s r=-0.52, p=0,016) and no correlation was found between the FA-values and the age of the healthy controles. Lithium and Lithium used in combination with other psychopharmaceuticals suggests a significant lower FA-value in euthymic I bipolair patients. Conclusion: During a euthymic episode a higher FA-value was observed, indicating a higher level of unidirectional diffusion of water in white matter. This may be evidence of an increased myelinsiation during the euthymic episode as reaction to the demyelinisation or dysmyelinisation during an episode of depression.
