Abstract

Introduction. Insufficient recruitment of patients in medical research is a common and widespread problem and can result in delay or even early termination of research projects. The MIRA study focused on women with heavy menstrual bleeding, comparing treatment using a levonorgestrel IUD or endometrium ablation, also encounters problems in recruitment of patients, particularly in primary care. The aim of this qualitative study was to identify both barriers and facilitators for patient recruitment by general practitioners for scientific research in general and in the MIRA study. Methods. Semi-structured interviews were conducted with a purposive sample of 16 Dutch general practitioners (GP’s), all participating in the MIRA study. The interviews were audio recorded and fully transcribed. Thematic analysis was performed by three independent researchers and was used to code the data and divide the discovered factors into several themes. After discussions in the research group, agreement on the overarching themes was reached. Results. Identified barriers and facilitators at the level of general practitioners were divided into eight themes. In particular, the investment in time and effort and the (insufficient) knowledge and awareness of the study were important barriers to patient recruitment. Additionally, attitude towards scientific research in general, characteristics of the GP (practice), usual care of the GP and protection of the patient, both which may lead to a preference in treatment, were likely to influence recruitment. Also the low incidence of the disease was frequently mentioned as a limiting factor for patient recruitment. GP’s mentioned that patient preference for treatment was a frequent reason not to participate in the study. Conclusions. In concordance with literature available in secondary care, recruitment of patients by GP’s remains difficult and depends on many factors. As noted by the GP’s, communication between the researcher and GP, as well as giving reminders about the study and a study design which takes the investment of the GP into account, appears to be important and could be influenced by the researchers. The attitude of the GP towards research in general could possibly be changed by implementing more education in the GP training programme. In order to be able to implement the results in general research, it is of interest to quantify in which extend the identified barriers form an obstacle to patient recruitment.
