Abstract

Giant cell arteritis (GCA) is a common vasculitic disorder affecting medium- and large-size arteries. To diagnose the disease a temporal artery biopsy is recommended. Histologically, a complex interaction between mainly dendritic cells, T cells and macrophages is seen and eventually causes luminal occlusion with ischemic complications. GCA is a disease of the elderly and occurs in an aged immunesystem. Important feature of this so called immunosenescence is a increase in CD16-positive monocytes. The main question was whether or not CD16-positive monocytes had a role in the pathogenesis of GCA. To answer this question immunohistochemistry was performed in temporal artery sections from patients with histological evidence of GCA. By immunological staining no monocytes were detected in the tissue, but CD16 was abundantly expressed by all kind of immunological cells, for example macrophages. CD16 was significantly more present in the adventitial layer from GCA patients compared to the control group and probably reflected an upregulated inflammatory state of the artery. Due to the fact that CD14+ monocytes are not present in the vessel wall, we could not show involvement of ‘aged’ CD16-positive monocytes in the pathogenesis of GCA at the level of the vessel wall. Since monocytes entering the vessel wall will transgress into macrophages future studies should be directed at aged macrophages.
