Abstract
Objective: Examining the efficacy of habitually worn shoes and ankle foot orthoses (AFO's) on gait kinematics and kinetics in children with low lumbar and sacral spina bifida (SB ). Design: Multiple Case Study Settings: gait analysis laboratory environment. Participants: 18 children with low lumbar and sacral level of lesion ( 5-16 yrs). Intervention: a nalysis of the gait when walking barefoot versus when walking with habitually worn shoes and AFO' s. Main Outcome Measures: ground reaction force (GRF) data, joint moments and walking velocity. Results: Results show only a obvious difference in ankle- and knee moments during terminal stance and in walking velocity in the children wearing a ventral solid AFO. Conclusions: It can be concluded that most of the habitually worn shoes and AFO's don't have an obvious positive effect on the kinematics and kinetics in gait in these children with SB. Only the v-AFOs showed an obvious effect by restoring the ankle stiffness and ensuring progression of the GRF to the forefoot ' whtch creates an external knee extension moment in mid stance and terminal stance.
