Effectiveness of Mindfulness-Based Cognitive Therapy (MBCT) to prevent a relapse of depression has been proven in several randomised controlled trials. Evidence also exists for the effectiveness of MBCT to treat other psychiatric disorders, amongst which bipolar disorder and anxiety disorder. This study examined the effects of MBCT on mental health complaints, quality of life and degree of mindfulness in a heterogeneous psychiatric outpatient population. Five diagnostic categories were examined: depressive disorder, bipolar disorder, anxiety disorder, adjustment disorder and ‘other diagnoses’. In total 143 patients were included (mean age = 45.3 year, SD = 10.4; 69% women; mean duration of illness = 10.4 years, SD = 9.5). Prior to treatment and immediately after participants completed the Dutch Short Complaints List (KKL), the World Health Organisation Quality of Life-Bref (WHOQOL-Bref) and the Kentucky Inventory of Mindfulness Skills-39 (KIMS-39). Multivariate F-tests for repeated measures were conducted while adding covariates sex, education and duration of illness. Patients with a depressive disorder (n=45), anxiety disorder (n=17) or ‘other diagnoses’ (n=25) experienced significantly less mental health complaints, experienced a higher quality of life and more mindfulness skills after the training. Patients with an adjustment disorder (n=14) experienced no reduction of mental health complaints, but did experience a higher quality of life and more mindfulness skills. Patients with a bipolar disorder (n=42) did neither experience a reduction in mental health complaints nor an improvement in mindfulness skills, but did improve on one of the five measured aspects of quality of life (i.e. physical health). These results show that MBCT can be effective in treating a heterogeneous psychiatric outpatient population, but that in this study MBCT was hardly successful for patients with bipolar disorder.
