Abstract
Background: Improvements in the cardiac surgery have led to a better survival rate for patients with congenital heart disease (CHD). This results in the need for studies regarding the quality of life (QOL) and exercise capacity of this population. This study targets patients with CHD after a pulmonary valve replacement (PVR). The primary aim is to compare the QOL of this group of patients with a healthy reference group. Secondary, independent determinants will be identified for QOL. Methods: We included 160 patients in a multi-centre cross-sectional observational study. The QOL is assessed with the RAND-36 Item Health-Related Quality of Life questionnaire, and is compared with former results of a Dutch healthy reference group. All of the performed tests are part of usual care, with an exception of the RAND-36 and an additional telephone questionnaire to determine i.e. current health status and sociodemografic characteristics. Results: Adult patients with CHD after PVR score significantly lower on QOL-domain general health (p<0,001), but score significantly higher on the QOL-domains: physical functioning (p=0,0057), mental health (p=0,0188) and bodily pain (p<0,001). The assessed effectsize is small, indicating a small clinical relevance. Independent determinants for general health are chronotropic index, NT-proBNP, employment status and NYHA classification explaining 14-32% of the variance. Conclusion: This group of patients reports similar or even better QOL scores compared with a healthy control group, except in the domain general health. The determinants found in this study could account up to 32% of the variance in the QOL-domain general health. Further research is needed to determine other important determinants for QOL.
