Abstract

Purpose: To determine the effectiveness of percutaneous balloon dilation and stenting of benign biliary strictures. Materials and Methods: Medical records of patients with benign biliary strictures in whom percutaneous transhepatic biliary drainage (PTBD) and balloon dilation was performed in the period of January 1, 1999, to December 31, 2011, were retrospectively reviewed. Conventional angioplasty balloons (4-10 mm) were used for PTBD. An internal-external biliary drainage catheter (8.5-12 F) was placed after balloon dilatation. Patients were scheduled for elective tube changes, if necessary combined with balloon dilatation of the stenosis, at 3-week intervals for of up to a minimum of 3 months. Results were assessed with cholangiograms. Results: 100 patients received a total of 136 treatments. The treatment was considered technical successful in 98% of the patients. Drains were left in situ with a median of 14 weeks. Complications were seen in 11 patients (11%). In 13 patients (13%), treatment was discontinued to proceed with surgical management. Of the 87 patients with follow-up, 11 patients (12.6%) developed clinically significant restenosis. Follow-up had a median length of 35 months. Probability of patency at 1, 2, 5, and 10 years was 96.3%, 93.4%, 88.0%, and 65.2%, respectively. Overall, 76 patients (76%) had successful management with PBBD. Patients with multiple treatments showed restenosis more often. A correlation between any variable and discontinuation of treatments has not been found. Conclusion: Percutaneous balloon dilation and stenting demonstrate good short- and long-term effectiveness as treatment for benign biliary strictures, and are indicated as the treatment of choice.
