Abstract

Objectives To determine the prevalence of rHOA at t=3 years in a RA cohort and to compare patients with and without rHOA regarding DAS28 response and medication use, in order to evaluate efficacy of antirheumatics for HOA. Patients and methods Patients with 3-year follow-up from a RA cohort were included. The radiographs of the hands at 3-year follow-up were scored according to the OARSI atlas and patients were classified as having rHOA when the derived K&L grade ≥2 in ≥3 joints. The two groups (rHOA=OA group and non-rHOA=RA group) were compared regarding DAS28 response, CRP and medication use over and at 3 years. The same was done for age-matched groups in a subgroup analysis. Results The prevalence of rHOA at 3 years was 34%. (59/175 patients) DAS28 mean and percentage DAS28 remission in time showed similar curves for the OA and RA groups. At 3 years, 75% of patients in the OA group and 62% in the RA group used methotrexate. (p-value=0,08) In the age-matched subgroups these percentages were respectively 80% and 61%. (p-value=0,06) The percentage cumulative biological users was 30% in the RA group and 15% in the OA group. (p-value=0,03) Between the age-matched subgroups however, this difference was annihilated. Conclusion This observational study suggests presence in this RA cohort of patients with HOA who fulfilled criteria for RA at baseline. They seem to have the same clinical course and response to treatment as those without HOA, which suggests beneficial effects of antirheumatic drugs on symptoms of HOA.
