Abstract

INTRODUCTION – Nowadays poor-quality kidneys have to be transplanted more often. It is desirable that the preservation of these kidneys will be improved. With the ability to induce hypometabolism, reduce reactive oxygen species (ROS) and to reduce apoptosis, hydrogen sulphide (H2S) appears to be a very promising therapy for preservation of these kidneys. AIM – To investigate the effects of H2S on metabolism, kidney function and kidney damage of a porcine kidney after 24 hours of preservation. METHODS – Twelve porcine kidney were examined in a closed perfusion circuit. The experimental group (n=6) repeatedly received H2S to a total amount of 72,8gr during preservation with a standardized 22,5 hours of hypothermic machine perfusion (HMP) herein. Afterwards this group was compared to a control group (n=6) during 2 hours of normothermic reperfusion. To be able to compare the groups, oxygen consumption was measured as reflection of metabolism, creatinine clearance and fractional excretion of sodium for the kidney function and ASAT and LDH in combination with histology for kidney damage. Measurements were analysed using linear mixed model analysis and the Students T-test. RESULTS – Between both groups no significant differences were seen for oxygen consumption (p=0,686), creatinine clearance (p=0,518), fractional excretion of sodium (p=0,522), ASAT (p=0,082) and LDH (0,358). Histologically no differences were seen as well. CONCLUSION – Both the experimental group and the control group function equally during reperfusion. Therefore, it is assumed that H2S does not damage the kidneys. However, the exact effect of H2S during preservation needs to be elucidated. H2S is still a very promising therapy for preservation. More research needs to be done to examine the effect of H2S on long term preservation of kidneys.
