Abstract

Introduction: The combination of an increasing prevalence of diabetes mellitus and more people having access to smartphones creates opportunities for patient care. Previous studies have demonstrated that the use of telemedicine leads to improved glycaemic control and simplifies care for both patients and health care providers. However, the question if the use of a mobile phone application also leads to an improved quality of life has never been investigated. The primary aim of the current study is to investigate whether the use of the Diabetes under control (Dbees), a digital diabetes diary, results in a better quality of life for patients with diabetes type 1 compared with the standard paper diary. Patients and methods: In this 3-month randomized controlled open-label trial, 64 patients with diabetes type 1 and access to a smartphone were assigned to the Dbees application (n=31) or the standard paper diary (n=33). Primary outcome was the quality of life measured by using the RAND-36 questionnaire. Secondary outcomes included diabetes-related distress, HbA1c, frequency of self-monitoring blood glucose and the usability of the diabetes application. Results: 64 Patients with a mean age (SD) of 36 (12) years; a diabetes duration of 19 (13) years and a mean HbA1c of 62 mmol/mol were included. There were no significant differences in the RAND-36 between both groups. However in the intervention group the general health subscale decreased significantly (p=0.02): from 64.4 (19.1) to 58.9 (21.2) compared to 58.8 (20.2) towards 55.6 (22.4) in the control group (p=0.13). Glycaemic control, diabetes-related emotional distress and self-monitoring blood glucose remained unchanged in both groups after three months, without significant differences between the groups. The intervention group reviewed the usability of Dbees with a 72 (20), on a range of 0-100. Conclusions: The short-term use of the Dbees application in the management of type 1 diabetes yielded no significant alterations in quality of life compared to routine care. The patients were satisfied with the use of Dbees and they considered the digital diary an improvement that needs to be developed further.
