Abstract

Cardiovascular diseases (CVD) are the most important cause of death in patients with type 1 diabetes mellitus (T1DM). Children and adolescents with T1DM and a high LDL-cholesterol concentration (dyslipidaemia/hypercholesterolemia) have an increased risk. In this study we investigated in children and adolescents with T1DM, the diagnostic and treatment options for dyslipidaemia, by performing a literature-study and a retrospective data-analysis in the University Medical Center Groningen. We included 210 persons with T1DM (112 men and 98 woman), aged 3-24 years (SD 4,8). In patients with dyslipidaemia (LDL≥2,6 mmol/l), we investigated if there were other risk factors for getting CVD, whether they got treatment and the reasons for (not) treating them. In this population, 41,9% (n=88) had dyslipidaemia. The patients ≥18 years had the highest mean LDL-c and HbA1c. 49% had two or more risk factors for getting CVD, 11,4% of the population with dyslipidaemia received a dietary advice and 5,7% had medication(statins). There was a delay in treating the dyslipidaemia, due to a lack of evidence for treatment in this population and efforts are focused on improving metabolic control. Based on studies in adults with T1DM, children with familial hypercholesterolaemia and guidelines in children with T1DM, we composed a flow diagram how to act in case of an increased LDL-c. As there have been hardly any studies performed in children with T1DM and dyslipidaemia suggestions for further research are given.
