Abstract
Objectives Patients presenting with sleep-related painful erections (SRPEs) suffer from deep penile pain during nocturnal erection, waking them up and disturbing their nights of sleep. SRPEs are associated with relatively long-lasting Rapid Eye Movement-sleep episodes. Sex-related erections are typically normal in these patients. This rare parasomnia is poorly recognized by different specialists. In this study, we aimed to gain more insight in respect of diagnostics and therapeutic options. Methods Data from a series of 24 consecutive patients, who presented with SRPEs at the outpatient clinic during 1996 to 2015 were retrospectively analyzed. Additional questionnaires were sent to complement data and information about follow-up. Results Mean age was 53 years (38-74). SRPEs proved neither to be associated with medical/psychiatric history nor with serum testosterone levels. The mean doctors’ delay was 3,5 years. In 14 of the 24 patients Nocturnal Penile Tumescence measurements were performed, showing relatively long episodes of nocturnal penile erection in most of them. Thirteen of the 24 patients were treated with baclofen (10-75 mg). In 10 of them complete remission was observed within a few weeks. Two of the three remaining patients noticed a slight improvement of SPRE symptoms and only one patient experienced no effect at all. After an average follow-up of 6 years, only 37,5% of the patients that have used baclofen were satisfied with regard to their SRPEs. The other ones (62,5%) were dissatisfied, mainly due to high relapse rates after discontinuing baclofen. Eleven of the 24 men were treated differently, with variable results. Conclusion This study confirmed a high doctors’ delay in SRPE patients. There was no association between SRPEs and medical/psychiatric comorbidity and total serum testosterone levels. Treatment with baclofen proved to be successful and safe on short term. Long term feasibility needs further investigation.
