Abstract

Introduction: There are more and more frail elderly people in Dutch hospitals, which is accompanied by the increase in specific syndromes and symptoms. The last years more guidelines have been developed for this, but it is not known whether these guidelines have been implemented in local hospital protocols. The aim of this study is to look to what extent up-to-date protocols for relevant problems in frail elderly people, namely sleep problems, delirium, dealing with dementia, behavioral problems and psysical restraints, are present. Methods: All Dutch hospitals were asked to fill in an online survey about the presence of protocols and to upload these protocols. All protocols are analyzed by an independent clinical researcher, who checked whether the recommendations from the guidelines were implemented in the protocols. Results: Of the 73 hospitals that were approached, 42 (58%) sent the available protocols. All hospitals had a protocol for delirium and physical restraints, 13 (31%) for dealing with patients with dementia, 4 (10%) for sleep problems and 2 (5%) for behavioral problems. 32 (78%) delirium protocols were based on the guideline while in physical restraints 24 (65%) protocols. Not one protocol mentioned all recommendations. Conclusion: All participating Dutch hospitals have a protocol for delirium and physical restraints, while hospital protocols for dealing with dementia, behavioral problems and sleep problems are very limited. The protocols for delirium and physical restraints are largely based on the guidelines but not all recommendations are reflected in the protocols and the implementation differs strongly per recommendation.
