Abstract

Introduction Complications following breast reconstruction lead to an important clinical problem. This retrospective multicenter study was conducted to investigate how many and what complications occur in the East of the Netherlands and how this compares to the literature. It was investigated whether there is a difference between an immediate and delayed reconstruction and between a reconstruction with an implant and autologous reconstruction. Furthermore, independent risk factors for the occurrence of a complication after breast reconstruction were investigated. Methods All breast reconstructions performed in the period January 2009 - July 2013 in the three hospitals in the East of the Netherlands were retrieved. Complicated reconstructions were analyzed and compared with a random sample of uncomplicated breast reconstructions to predict independent risk factors for the occurrence of a complication. Results In 257 of 1258 breast reconstructions (20.4%) a complication occurred. Multivariate logistic regression analysis resulted in two independent predictors for the occurrence of a complication: type and timing of reconstruction. Other possible risk factors were not found to be independent predictors. Conclusion Types of complications and the overall complication rate after breast reconstruction in the East of the Netherlands correspond to the literature. Immediate reconstruction and autologous reconstruction are independent predictors for the occurrence of a complication. Other independent risk factors have not been demonstrated in this study. It could be considered if an immediate reconstruction would prevent extra surgery and whether autologous reconstruction provides better results compared reconstruction with an implant. These results may support the preoperative choice for type and timing of breast reconstruction.
