Abstract
Background: Sacral NeuroStimulation (SNS) is an effective treatment for fecal or urinary incontinence, urinary retention and constipation. The first stage lead placement (FSLP) of this procedure is a lead placement in the S3 foramen to perform a test stimulation period of a few weeks. The second stage is the placement of an implantable pulse generator (IPG). Only patients with a >50% improvement of symptoms after the first stage proceed to the second stage. So, effectiveness is usually given in subjective percentages of symptomatic improvement, also in the long term. This study aims to investigate the results and effectiveness of the SNS procedure in other terms, like Quality of Life (QoL). Material and method: In this single center retrospective study, a total of 141 patients received a FSLP, of whom 77 proceeded to IPG implantation. All patients received QoL-questionnaires and results were compared with pre-procedure QoL data (when present). Among the questionnaires used are: ICIQ-QoL, pac-QoL, FiQoL and a qualitative satisfaction questionnaire. Results: Of all 141 patients (mean age: 55 years) included (73 NOR; 43 UI; 9 FI; 9 constipation; 7 other) 108 (76,6%) participated in the questionnaires. Mean follow up was 56 months. After follow up, Quality of Life rose in the implantation group, while it decreased a bit in the non-implantation group. The nvog-QoL2 parameter showed a significant result in Quality of Life, in favor of the implantation group (p=0,039). The other results in Quality of Life were not significant. In the implantation group, self reported improvement of >50% was high (80%) and satisfaction was also high (90%). Conclusion: Although this study showed a high satisfaction and a great self reported improvement with SNS treatment, this could not convincingly be reproduced in terms of Quality of Life. Safety, efficacy and satisfaction were comparable with other centers.
