Abstract

Background: Malnutrition is an important health problem for patients admitted to the hospital or intensive care unit (ICU). Malnutrition during hospitalization results in an increased morbidity and mortality. This study aims to compare the anamnestic short nutritional assessment questionnaire (SNAQ) with the phase angle (PhA) in patients admitted to the ICU. Objective of the study is to assess how many patients are malnourished at admission to the UCI and whether these patients differ from patients without malnutrition. Does a correlation exist between the SNAQ and PhA? Methods: A prospective observational study in patients admitted to the ICU of the MCL. At admission the SNAQ-score and the phase angle are measured and admission data (indication for admission, Body Mass Index, Acute Physiology And Chronic Health Evaluation II (APACHE II)) and clinical data (co-morbidity, laboratory values, caloric and protein intake) are collected. Results: 299 patients were included, of which 66% were male, with an average age of 66 ±12 years and a median APACHE II of 14 (IQR 11-17). According to the SNAQ, 15.7% of the patients was malnourished at ICU-admission and 35.5% with PhA. Malnutrition was associated with a prolonged hospitalisation and mortality. PhA was significantly correlated with the SNAQ (P<0.001). The phase angle is a prognostic marker of hospital mortality, independent of severity of disease. Conclusions: SNAQ as well as phase angle are good methods to assess malnutrition. Besides malnutrition, phase angle also signalizes changes in body composition and also is a prognostic indicator of length of hospital stay and mortality, independent of disease severity.
