Abstract

Background: It is still unclear which factors contribute in medical unexplained symptoms, how these factors correlate and which factors are most important in perpetuating medically unexplained symptoms (MUS). This research is going to survey these various issues to improve the treatment of patients with medical unexplained symptoms. Methods: Perpetuating factors in medical unexplained symptoms are obtained by interviewing 12 therapists with extensive experience in treating patients with medical unexplained symptoms. A total 660 statements were obtained, which are reduced to a total of 99 factors. Thereafter, 28 Therapists with an average of 9,5 years of treatment experience,individually sorted 99 selected factors by content per card sorting task. The Best Merge Method was applied to these sorting. The importance questionnaire is fulfilled by 19 therapists how they think about the importance of the 99 factors on a scale from 1 to 10. Wilcoxon Signed-Rank Test was used to determine the difference in importance between the clusters. Results: The interviews resulted in 99 core factors that might perpetuate medical unexplained symptoms. The card sorting task and cluster analysis resulted in 10 clusters: Environment, health care, body focus, personality, mentalization, emotion, cognitive, body focus, lifestyle and fear of movement. The median (=7,8) of the body orientation cluster is significant larger (p<0,001) than the average median (= 6,9) of all clusters together. The remaining nine clusters do not significantly deviate from the average median. Conclusion: The cluster body focus was considered the most important in perpetuating medically unexplained symptoms. Follow-up research with more professions and MUS-patients is needed to survey the consistency of perpetuating factors and the importance of perpetuating factors.
