Abstract

Introduction The rate at which the gound reaction force (vGRF) increases in vertical direction (Loading Rate; LR) seems to be of importance in the onset of running injuries. Runners may experience fewer injuries when the LR is reduced by 10-15%, feedback focused on gait retraining appears to provide such reduction. There are two different types of gait retraining feedback, the first one depends on the information from a person’s own body (internal focus) while the other feedback depends on information from outside the body. The question is: what kind of gait retraining feedback is most effective in lowering the LR. Clarity about this can provide targeted preventive instructions while running. Material en methods In this pilot study, 31 recreational runners were randomly divided into two groups. The feedback they received depended on the grouping. Group1 received feedback with an internal focus ‘try to soften your landing’, Group2 received feedback with an external focus ‘try to reduce the sound of your landing’. After collecting the LR from the basic running gait (T0) on a treadmill the effect of the feedback was measured after a 10 minutes run (T1) in which subjects received the feedback four times verbally. To determine the effect of the feedback after one month, subject where asked to come back after they integrated the feedback in their own training for one month (T2). The primary outcome measure was the LR. Differences in time and between groups ware tested non-parametric. Results In the internal focus group (T0: 84,6 (Q1:68,8-Q3:96,5), T1: 78,8 (Q1:58,7-Q3:97,0) , T2 72,5 (Q1:59,2-Q3:97,3)) there were no differences between the measurement points (P=0.223), in the external focus group there were differences between T0 (89,3 (Q1:68,4-Q3:118,8)) and T1 (66,6 (Q1:51,2-Q3: 95,3)) (P<0,001) and also between T0 en T2 (66,6 (Q1:51,8-Q3:97,0)) (P=0,001). The decrease in LR was significantly different between the two groups at T1 (5,8; Q1 -8,7 Q3:19,3) vs. 21,6 (Q1:11,6 Q3: 29,6), P = 0.017). Level of significance = 0.05, after Bonferroni-correction: P<0,0125. Conclusion Feedback with an external focus provides a significant decrease in the LR. This feedback reduced the LR more effective than feedback with an internal focus. Discussion A higher Duty Factor (DF) can indicate a shorter flight phase or longer contact time. Further research should show whether a higher DF as in this study can provide a lower LR. Because the feedback with an external focus proves also to be more effective in other sports like football and volleyball. The application of the feedback with an external focus can possibly protect a lot of runners.
