Abstract

Background: Physical activity is important for COPD patients. Physical active patients report less dyspnea and better physical capacity. Also the number of exacerbations and the mortality is lower for the active patient. Yet older and especially COPD patients move less than recommended. Objective: To investigate the effect of a ten week telecoaching program to enhance physical activity in COPD patients. Methods: 28 patients (58% male, median age 64, median pack years 30, median Tiffenau-index 54.4) were randomized. The intervention group received a coaching program consisting of the Fitbug pedometer, and smartphone with the Fitbug and Linkcare app. Before starting the intervention the activity pattern of the patient was identified in a motivational interview. During the 10-week intervention period patients received an activity goal, which was raised weekly. At baseline and after 10 weeks spirometry and functional capacity measurements were performed and health related quality of life, dyspnea and anxiety and depression questionnaires were administered. Results: After the intervention, the patients in the both groups showed diminished physical activity, with no significant difference between the groups. The physical capacity showed an increase (+12m) in the intervention group and decrease (-16m) in the control group (p 0.072). However, the difference was smaller than the minimal clinical important difference. The questionnaires showed no differences between the two groups, except for the COPD Assessment Test, where the score improved significantly for the control group (-1 versus +3 for the intervention group)(p 0.02). This difference was greater than the minimal clinical important difference. Conclusion: Contrary to expectations the telecoaching program showed no impact on the physical activity of COPD patients. Further studies could aim to include more patients, so the power is sufficient, and to have a longer study period, to diminish the influence of the winter weather on the activity patterns of the COPD patients.
